Summary:

This tool takes groups through the goal-setting process, helping shift their focus away from the larger community change they are hoping for to specific, measurable, achievable goals for their individual ministry. It’s comprised of a set of worksheets, each one helping participants identify desired outcomes in several areas of change (spiritual, relational, health, social/economic, environment/structures, and others).

Also included is a sample worksheet completed for a church with a ministry to at-risk kids.
