Grace Hospice provides a team-approach to patient and family centered care that optimizes quality of life by anticipating, preventing, and treating the physical, social, emotional, intellectual, and spiritual needs of our patients and families. We focus on enhancing the quality of life and our volunteers play a key role in our mission. Grace Hospice ensures that our patients will spend their final days of their journey in comfort and with dignity.

Grace Hospice Cleveland East covers Eastern Cuyahoga County, Lake, Geauga and Ashtabula Counties.
