Application Procedures: 

Applicants must have the following items in their proposals:  Background Information about the organization, project budget, organizational budget, alist of foundation and corporate sponsors, a board of directors and/or a trustees list, the organizations most recent annual report, as well as recently audited financial statements, the most recent Form 990, and an IRS 501 (c) (3) determination letter.
